Sorva Healthcare Messaging Program.

Sorva sends transactional SMS messages to patients to facilitate healthcare referral coordination. Messages may include scheduling links, follow-up reminders, appointment confirmations, and referral status updates.

Message frequency varies depending on the status of your referral and scheduling activity.

Message and data rates may apply depending on your mobile carrier and plan.

End users opt in by providing their phone number during a healthcare referral, provider intake process, or Sorva-hosted scheduling workflow and agreeing to receive SMS communications related to their care.

You may opt out at any time by replying STOP to any message. After opting out, you will no longer receive SMS communications from Sorva.

Messaging services are supported on most major U.S. carriers but may not be available on all carriers.

Your information will be handled in accordance with our Privacy Policy.

Sorva reserves the right to modify these Terms at any time. Updates will be posted at sorvahealth.com/terms. Continued use of the service constitutes acceptance of such changes.
